Eligibility

As a full-time status PT Solutions team member (defined as 30 hours/week), you and your eligible dependents can enroll in benefits if you work at least 30 hours per week. Your eligible dependents include your legal spouse and your children up to age 26.


New Team Members

For new hires, benefits are effective on the first day of the month following 30 days of employment. You may only change coverage during annual open enrollment or within 30 days of a qualified life event.

You may have an orientation with Human Resources to review all your benefit options, timing and deadlines.

Qualifying Life Event

Certain life events, such as the scenarios below, provide you a 30-day window to elect benefits or change your covered dependents with the appropriate documentation.

  • Marriage, divorce or legal separation (Marriage certificate/court order)
  • Birth or adoption of a child (Birth certificate/hospital certification/adoption records)
  • Commencement or termination of adoption proceedings (Adoption records)
  • Loss of coverage at age 26 (Notice of termination from parental plan’s carrier or employer with name and effective date of termination)
  • Death of a dependent (Death certificate)
  • Loss or gain of other health coverage for you, your spouse and/or dependents, including loss of parental coverage at age 26 (Confirmation or letter of loss of coverage with name and effective date/benefit election summary)
  • Change in employment status
  • Change in residence due to an employment transfer for you or your spouse
  • Change in Medicaid/Medicare eligibility for you or a dependent (Notice from Medicaid/Medicare)
  • Receipt of a Qualified Medical Child Support Order (Court order)

You must contact Human Resources within 30 days of the event if you want to make a change to your benefit elections. Qualified Life Event changes initiated or received after the 30-day window may be denied. Life Event elections cannot be approved until the documentation outlined above is submitted. Update your benefits and upload required documentation online via UKG > Manage My Benefits.
